Program profiles are provided in alphabetical order by State and city. Indexes by organization name, addiction and disorder, and by treatment method are provided. Each entry contains the organization, facility, or program name, address, contact person, and telephone number. Listings may also include "hotline" number, facsimile number, date established, parent organization, affiliations and accreditations, and licensure. The effectiveness percentage is reported for any evaluation done by outside agencies. Other information pertinent to evaluation is the evaluation methodology used and the effectiveness of the program in preventing addiction, relapse, or related problems. Other information provided may include the addictions or disorders treated, treatment methods, program setting, other client services, staffing, groups served, ownership, referral policies, admission requirements, fees, and followup.
