The successful completion of a self-administered questionnaire can only occur when the respondent is convinced that the questionnaire is not "junk mail" and, when opened, that it is worth the time and effort required for participation. The outside envelope must convey the importance of the contents; this is done through the postage stamp, name and address of the prospective respondent, and the return address. Survey cost factors can also be manipulated to maximize returns. The author recommends ways to lay out the questionnaire, pose the questions, print the survey, send follow-up mailings, and monitor returns. 16 references and 1 appendix
