Standard notification and reporting procedures of major incidents have been established in the policy and procedures manual of the Idaho State Board of Corrections. The policy is designed to facilitate improved communications, management, and research.
Abstract
Major incidents include, inter alia, offender disturbances and work stoppages; death or serious injury of an inmate, staff member, or community member; firearms incidents; escapes and attempted escapes; use of chemical agents, taser, or similar technology; and any offender-connected activity requiring notification for purposes of criminal prosecution of an offender, visitor, or other person. This procedure describes the steps necessary to report major incidents through the chain of command to the Office of the Director. Every incident, regardless of its nature, must be reported to the facility head, administrator, or bureau chief; each division should have in place its own procedures for reporting all incidents.
