Chronic care clinics are responsible for screening, identifying, and monitoring patients with chronic illnesses in order to initiate appropriate treatment regimens which will promote health and prevent complications. The clinics also aim to provide patient education and counseling to encourage patients to practice healthy behaviors. The clinics include the following: cardiovascular, diabetes mellitus, gastrointestinal, hypertension, infectious disease, seizure disorder, pulmonary, and tuberculosis. There are also men's and women's wellness clinics to serve the inmate population. These protocols cover intake, medical evaluation, diagnosis, follow-up, medication, and discharge procedures.
