A medical suspected-sexual-abuse form was created to serve as a template for data collection. The clinical and legal needs for the medical record were defined, and ideas used in forms by other programs were incorporated. A computer data entry form was created to complement the medical form. Sections of similar information were clustered on the medical and the data entry form. Screening questions were used to condense a large amount of information. An intake form was filled out by the clinical nurse specialist, nurse practitioner, or social worker who received the initial phone call for an appointment. Subsequent to the initial visit and after the psychological, social, and medical evaluation, a flow sheet of the psychological profile, checklist test results, and laboratory test results were filled out by the individuals who performed or ordered the tests. The medical form was completed by the pediatric nurse practitioner. All of the forms were reviewed by the Child Abuse Referral and Evaluation (CARE) medical director, a pediatrician, and information was entered on the three-page data entry form. The data were entered into the computer by data entry personnel. This approach to data management efficiently uses clinical personnel to serve as data collectors. 2 tables, 2 figures, 6 references, and data entry form
