This packet contains follow-up evaluation forms for victim/survivor programs, batterer's group program, and group program for children of battered women.
Abstract
The closing summary form for participants in a victim/survivor group contains such information as dates of intake, completion of group, last in-person contact, and closing; status of client in the treatment sequence at the time of last contact; reason for termination; services received; referral to other services; and experiences of threats or violence since entering the program. A similar closing summary form is provided for the abuser who is in treatment. The follow-up interview schedule for victims/survivors contains interview questions pertinent to the abuse, along with the nature and status of the interview. A victim/survivor-follow-up- feedback-to-therapist form contains questions related to what the participant thought about the program and any changes that may have occurred in the victim's/survivor's attitudes and behavior due to the program. A similar form for the abuser is provided. A Children's Program Evaluation form is to be administered to the battered woman and pertains to her perceptions of how the program has helped her children. Also included are closing summary forms for the counselor or therapist to complete when the client has completed services.
