The report is divided into 10 sections: (1) Frequency of Unusual Incidents in 1999; (2) Annualized Rate of Unusual Incidents; (3) Unusual Incidents by Correctional Facility; (4) Weapon Use by Inmates; (5) Inmate Role in Unusual Incident; (6) Inmate Deaths; (7) Location of Unusual Incidents; (8) Inmate Assaults on Staff and on Inmates; (9) Injury to Staff in Unusual Incidents; and (10) Type of Force Used to Resolve Unusual Incident. There were 6,887 unusual incidents during 1999, 96.7 incidents per 1,000 inmate population. In maximum security facilities the rate was 170.8 incidents per 1,000 inmate population. Inmates used weapons in 34.7 percent of unusual incidents. There were 194 inmate deaths in 1999, including nine suicides. An additional 174 deaths were due to natural causes and 11 deaths were pending determination of cause. There were 755 inmate assaults on staff and 1,310 inmate assaults on inmates. Most unusual incidents were resolved without injuries to staff. Department employees used force in 15.0 percent of unusual incidents, most often in inmate assault on staff incidents.
