This document contains the form for assent to participate in research activities and the questionnaire administered to students during the national evaluation of the program entitled, “Teens, Crime, and the Community and Community Works” (TCC/CW), which is a national school-based violence prevention program.
Abstract
In the student questionnaire, one set of questions solicits demographic and family information. Another set of questions focuses on students’ perceptions of the degree to which various issues pose a problem in the school and neighborhood, such as bullying, racial conflict, theft at school, and various types of vandalism. Another set of questions address students’ perceptions of themselves in terms of attitudes and behaviors, such as often acting on the spur of the moment, preparation for the future, focus on immediate gratification, liking to engage in risky behaviors, and sensitivity to others. A set of questions asks students about their level of fear in various situations. Another set of question queries students about the likelihood of various behaviors when they are with friends, followed by a set of questions on the student’s opinions about a number of behaviors, such as calling or pertinent to communication with family members, telling lies, fighting, and shoplifting. Other question sets address attitudes and behavior upon being angry/upset with someone, level of guilt about various specified problem behaviors, attitudes toward success and the future, feelings of self-worth, the personal risk for various victimizations, school experiences, peer behavior, involvement in community activities, and crime-prevention behaviors.
