Abstract
THE FILING OF SUCH REPORTS IS REQUIRED FOR ALL ACCIDENTS INVOLVING DEATH, INJURY, OR DAMAGE OF MORE THAN $200 UNDER SECTION 603 OF THE NEW YORK STATE VEHICLE AND TRAFFIC LAW. DEFINITIONS, CAUTIONS, AND PROCEDURES FOR FILLING IN EACH COLUMN OF THE FORM ARE GIVEN. THE FORM PROVIDES FOR THE COLLECTION OF INFORMATION PERTAINING TO LOCAL CODES, DRIVER/OWNER INFORMATION, VEHICLE AND ACCIDENT DAMAGES, ACCIDENT LOCATION AND DESCRIPTION, TICKET/ARREST DATA, INJURY DATA, PHYSICAL AND EMOTIONAL STATUS OF VICTIMS, ACTION TAKEN FOR INJURIES, TRAFFIC CONTROL DATA, LIGHT CONDITIONS, ROAD CONDITIONS AND CHARACTER, WEATHER, CONTRIBUTING FACTORS, DIRECTION OF TRAVEL, PREACCIDENT VEHICLE ACTION, AND TYPE OF ACCIDENT. PROCEDURES FOR MAKING REPORT CHANGES ARE OUTLINED. A FORM INDEX AND A SUBJECT INDEX ARE INCLUDED.
