This is an anthology of official forms used in Connecticut correctional institutions and exercises for teaching and learning adult basic skills necessary to complete the forms and function competently within the correctional structure.
Abstract
The forms provided are the orientation interview form, the classification waiver form, the inmate medical screening form, the inmate's inside movement pass, the money and property transfer request form, child visit authorization, the voluntary interview statement, store chits, and the form for request for action by staff. The nine units of the presentation are introduced by an explanation of the form to be studied. The skill lessons associated with the completion of each form focus on an understanding of the language used on the form, the use of oral and written language appropriate for communication within a correctional administrative climate, and specific guidance on how to complete the form. Each unit provides space for exercises of dictation, spelling, and short written compositions. A creative writing and reading exercise concludes each exercise to encourage self-expression. This anthology is appropriate for use in remedial programs.
