This crime report form is designed to collect information on the initial investigation, followup investigation, and forensic laboratory examination of violent crimes.
Abstract
Part 1, covering the initial investigation, solicits information on case management; descriptions of victim, suspect, arrestee, or unknown person; the victim's body (as found); cause of death and trauma; weapons used; and the criminal methods used in the offense. Part 2 provides information on the victim, the suspect's background, weapons used, physical evidence recovered, and the criminal method used in the crime. Part 3 presents information on forensic laboratory examinations, serology and toxicology, firearms and toolmarks, and hairs and fibers pertaining to the case. Instructions for completing the form are provided.
