For youth with physically or intellectually disabling conditions, this transition is often hindered by barriers such as economics, lack of preparation, attitudinal problems, lack of available facilities, lack of training, and unfamiliarity with professionals who can facilitate appropriate and achievable levels of independence. Therefore, these youth need additional support and assistance. In addition, they often require services from several different agencies. These services may include medical services, psychological services, vocational services, life-skills development, housing placement, sexuality counseling, language skills, and other services. Providing these services requires thoughtful individual planning and the conscious allocation of resources. 6 references.
