Reference documents on background, mission, and scope of the investigation include a report on a technical assistance visit conducted at the facility April 14-23, 1993. The report assesses the riot/hostage situation and operational levels throughout the living units of the institution that were not involved in the disturbance. Another reference document on background, mission, and scope of the investigation is an accreditation report on the Southern Ohio Correctional Facility (SOCF), which was awarded by the American Corrections Association on January 20, 1992. Other reference documents pertinent to background, mission, and the scope of the investigation are a National Institute of Corrections audit of SOCF and response in 1991; a report of the Select Committee to Investigate Conditions at SOCF, 1990; a report of the investigation of SOCF by the State Highway Patrol and responses, 1990; the J.D. Henderson Report and response, 1985; and a paper on enhancing SOCF security. Reference documents on the environment include CIIC quarterly information and Use-of- Force Logs for 1992 and 1993.
