The intervention model is implemented by a prevention case manager (PCM), who works with 4-5 women prior to release and 12-15 women in the community after their release. Through weekly case-management meetings with inmates a few months before their release, the PCM completes a needs assessment that culminates in a client-centered discharge plan. The inmates also participates in twice-weekly self-development group meetings that assist inmates in developing goals; problem solving and management skills; and knowledge about substance use, sexual health, interpersonal relationships, and employment. The PCM also partners with the child welfare worker to prepare the family for reunification after the inmate's release. The PCM acts as an advocate for the inmate mother and works with service providers who have been responsible for any of the inmate's children in order to help the mother be an effective parent after her release. The PCM continues to work with the client for 1 year following release to refine service plans and provide ongoing support. 11 references
