Model forms for establishing efficient paperwork flow management in security and safety operations are presented.
Abstract
Forms are standardized instruments designed to record and transmit data in a clear, organized, coherent, and complete manner. The sample forms included in this text provide a variety of examples indicating the basic elements included in forms actually in use at many safety/security agencies throughout the country. Three basic types of forms are discussed: check-off forms in which the writer selects the best response to a given set of criteria; fill-in forms which require the responder to write in a digit, letter, symbol, word, sentence, or short paragraph; and narrative forms in which the major portion is left blank for the responder's detailed narration. The sample forms can and should be customized to the individual needs of the particular organization. The sample forms included generate the following types of reports: (1) administrative, (2) investigative/information gathering, (3) identification and access control, (4) safety and inspection, and (5) miscellaneous. Chapters are dedicated to such basic types as Personnel Forms, Orientation and Training Reports, Daily Activity Reports, Accident Reports, Motor Vehicle Reports, Shipping and Receiving Forms, Fire/Safety/Bomb Reports, and Inspection Reports.
